ContrastConnect has released a new guide explaining AB 460 in California, covering implications for medical imaging and virtual supervision.

-- With Assembly Bill 460 (AB 460) officially being signed into law in California as of January 1, 2026, the state has aligned its definition of “direct supervision” with the Centers for Medicare & Medicaid Services’ (CMS) rules, allowing for virtual supervision of level 2 diagnostic imaging procedures. ContrastConnect has published its latest guide to cover the specifics of the changes, implications for imaging centers, and how facilities can begin integrating the virtual supervision model into their operations.

Prior to AB 460, contrast studies in California required a radiologist to be on-site before the procedure was performed. While physical presence was once considered the only reliable method to ensure patient safety during contrast media administration, ContrastConnect’s guide says that this framework resulted in inefficient deployment of radiologists across multiple sites; consequently, patient appointments were often limited to narrow timeslots, creating scheduling bottlenecks that led to delays in care and potentially worsened outcomes for severe illnesses. Rural and underserved communities were disproportionately impacted by these restrictions, often forcing patients in these areas to travel long distances to receive timely care.

However, under AB 460, remote supervision methods now satisfy the “immediate availability” requirement mandated by federal guidelines. This enables radiologists to supervise contrast-enhanced procedures remotely through real-time audiovisual communication technology, provided they have access to pertinent patient information and other required documentation.

ContrastConnect’s resource adds that remote supervision enables medical facilities to perform contrast-enhanced diagnostic exams without a radiologist on-site, allowing for more flexibility in scheduling. This potentially improves both patient access to care and work-life balance for radiologists, the latter of which can reduce burnout—a factor that contributes to high rates of attrition across the field.

In the absence of the aforementioned constraints, imaging centers will also be able to extend operating hours and serve more patients, increasing their revenue. As a result, ContrastConnect believes the virtual supervision model is highly scalable, provided it is implemented properly.

Imaging groups interested in adopting virtual supervision can partner with ContrastConnect, which will provide the infrastructure, training, and documentation necessary for compliant implementation.
